Surrounded by vineyards, with an incredible view of the mountains and the lake, the Hôpital de Morges welcomes you to a warm and dynamic setting.* We are looking for the Hôpital de Morges, a: Care Coordinator for chronic patients and complex situations M/F at 80% to 100% YOUR MISSION In collaboration with family doctors, you will autonomously ensure the follow-up and coordination of patients suffering from chronic illnesses or presenting complex medico-psycho-social situations. Thanks to your sense of organization and your spirit of initiative, you anticipate the needs of patients and contribute actively to the improvement of their care pathway. You play a central role in the organization of this pathway in order to guarantee the continuity, quality, and economic efficiency of the services. YOU POSSESS • A CRS-recognized nursing diploma or a federal certificate of Ambulatory Medicine Coordinator, clinical orientation. • An ability to juggle between multiple interlocutors and situations while maintaining a patient-centered approach. • An excellent sense of organization allowing for efficient prioritization and managing a large patient portfolio. • The capacity to work autonomously while implementing an interprofessional approach. • A good command of IT tools and clinical documentation. • A good knowledge of the health network and medico-social actors. • Excellent interpersonal and communication skills. YOUR OBJECTIVES • Ensure, autonomously and in collaboration with family doctors, the coordination and support of patients suffering from chronic diseases or presenting complex medico-psycho-social situations. • Evaluate the needs of patients and identify medical, psychosocial, organizational, and economic issues. • Coordinate interventions between the different professionals involved (doctors, nurses, therapists, social services, caregivers, etc.). • Ensure regular follow-up of patients, anticipate risky situations, and facilitate access to appropriate resources and services. • Develop a care partnership with patients to strengthen their autonomy, their active participation in decisions, and their adherence to treatments and recommendations. • Participate in the continuous improvement of processes, the quality of care pathways, and ensure rigorous documentation of interventions.
