Social Worker BSc 80% - 100%

The Case Management team consists of Case Managers and Social Workers. It accompanies inpatient and outpatient patients as well as their relatives in demanding social and psychosocial issues. This includes, among other things, topics such as adult protection, social insurance, financial and social issues, as well as the organization of tailor-made outpatient or inpatient aftercare solutions.

Your tasks ###

• Counseling and accompanying patients in complex social issues as well as taking over case management
• Organization of individual and demand-oriented aftercare and discharge solutions, as well as mediation of material aid if required and triage to appropriate specialist centers
• Interdisciplinary cooperation with doctors, nursing professionals, and other professional groups to ensure coordinated patient care
• Practical training of students of Social Work (BSc) or willingness to complete the corresponding further training

Your profile ###

• Completed studies in Social Work (BSc FH)
• Ideally additional training in healthcare (e.g., FaGe, Pflege HF or FH)
• Training as a practical instructor (Praxisausbildner/in) or willingness to complete this
• High social and counseling competence as well as enjoyment of interdisciplinary cooperation
• Independent, structured, and solution-oriented way of working
• Interest in working in the dynamic environment of an acute care hospital

Working hours ###

The standard operational working time for a full-time position is 42 hours per week (excluding doctors). Annual working time applies, which ensures the highest possible flexibility in the design of working hours. Shift requests are implemented where possible and part-time employment is possible in many functions - also in leadership positions.
