Social Worker 50-60%

Mission:

• Ensure all social follow-up at the homes of people presenting a state of physical or mental dependency or a disability.
• Act as the "case manager" for clients and evaluate their needs.
• Actively participate in a multidisciplinary manner in the organization and follow-up of the care project defined with the client.

Profile:

• Holder of a Bachelor's in social work with an orientation in social services or a title deemed equivalent.
• Experience, if possible, in the social or medico-social field.
• Good knowledge of social insurances and the Vaud medico-social network.
• Ability to work autonomously and in an interdisciplinary team.
• Listening and negotiation skills.
• Holder of a driver's license and owning a vehicle (possibility of using a professional vehicle).
