REFERENT NURSE AT 80 %

YOUR MISSION

As a referent nurse, your role involves an advanced mastery of specific skills:

• Personalized planning: Propose, organize and coordinate intervention plans adapted to clients, ensuring their satisfaction and respect for their needs.
• Rigorous follow-up: Supervise and regularly re-evaluate situations, coordinate actions essential to safety and continuity of care.
• Document management: Responsible for the management of OPAS files and the updating of client files in accordance with directives.
• Interdisciplinary collaboration: Work closely with the entire network (CMS team, family caregivers, doctors, partners) to ensure optimal care.

YOUR PROFILE

• You hold an HES nurse diploma or an equivalent title, reinforced by significant experience of 2 years, preferably in home care.
• You are versatile, attentive and autonomous; possess excellent interpersonal and negotiation skills and show a pronounced interest in maintaining home care.
• Your sense of organization, your ability to anticipate needs and to adapt to any type of situation and your competence in stress management are essential qualities.
• You hold a driver's license and, ideally, have a personal vehicle.
