Day Nurse (HES) at 80% We are looking for a day nurse to strengthen its health services. Your mission is to ensure the health responsiveness of several living groups. You guarantee the continuity and quality of nursing care, develop prevention and health promotion actions, and work in close interdisciplinary collaboration with socio-educational staff and network partners, in compliance with the institutional concept. Mission: Health responsiveness and continuity of care: - Be the reference person regarding health for several living groups - Ensure the continuity of preventive, curative, palliative and delegated nursing care - Identify critical situations and respond to them in an appropriate manner Prevention and health promotion: - Implement prevention, health promotion and screening actions - Promote hygiene measures and health-favorable behaviors - Inform and sensitize teams and the people welcomed to health issues Interdisciplinary work: - Collaborate closely with socio-educational staff in support - Participate in interprofessional meetings and contribute to common reflections - Work with external partners (doctors, hospital, CMS, etc.) Monitoring and quality: - Keep medical records and nursing writings up to date - Develop health summaries and care projects. Profile: * HES or ES nursing diploma, or a title deemed equivalent * Strong interest in prevention, health promotion and community health * Experience in community health, an asset * Ease in interdisciplinary work with socio-educational teams * Sense of responsibility, autonomy and analytical capacity * Human, collaborative and solution-oriented approach.
