For our Home Treatment, we are looking for you

These are your tasks

• Outreach social counseling and support for adult patients as well as their social environment
• Inter-institutional network work (authorities, offices, residential facilities, outpatient services, legal guardians)
• Mediation and triage tasks as well as work with relatives at home
• Participation in the further development of social counseling services

What you bring with you

• Degree from a university of applied sciences for social work (BSc)
• Competence in interprofessional cooperation and good communicative as well as social skills
• High level of personal responsibility as well as an independent way of working
• Professional experience with mentally ill people
• Resilient and flexible personality with an interest in the issues of mentally ill people
• Driver's license Cat. B
