As a supplement to our existing specialist responsible for caring relatives, we are looking for another one, immediately or by arrangement Dipl. Pflegefachperson HF/ FH with additional function as specialist responsible for caring relatives Your versatile tasks as a nursing professional are: • You are active in direct nursing and take on the consultation and guidance of customers, relatives and employees in complex nursing situations • You perform needs assessments using the InterRAI instrument (RegioSpitex Limmattal covers the costs for the InterRAI further training) • As case manager, you are responsible for the planning and implementation of the nursing process for your customers • You play an active role in cooperation with the caring, interdisciplinary network and coordinate the corresponding services for your customers Your additional tasks together with a colleague as specialist responsibility for caring relatives are: • You lead, advise, accompany and promote the caring relatives in technical and social situations, conduct regular status meetings, ensure the flow of information and organize demand-oriented training. • You design the entire personnel process in cooperation with HR - from recruitment to induction to offboarding. • You ensure that the caring relatives complete the required SRK course within the specified period, and accompany them in implementing what they have learned in practice. • You take over the case management of the customers of caring relatives including the administrative steps, coordinate the deployment planning and ensure quality on site. You bring with you: • You have a degree as Dipl.
