Somatic Reference Nurse (60-100%)
Your mission
As a somatic reference nurse, you ensure the role of case manager. You perform and delegate quality services in order to guarantee the health, safety, and well-being of clients at home. At their place of residence, you define with them the solutions most adapted to their situation, thus developing a privileged relationship of trust.

Your main responsibilities
Evaluate the clinical needs of the person and build their personalized care plan in partnership with them and their entourage.
Go to the homes of the clients and provide the services planned in the intervention plan.
Ensure network management and collaborate closely with home care and nursing staff while ensuring the quality of services.
Document follow-ups in the computerized file according to institutional directives, thus guaranteeing continuity of care.
Create and maintain lasting links with partners of the regional medico-social network (professionals, volunteers, user groups).
Ensure the role of referent in the global follow-up of situations: evaluation, orientation, advice, support in administrative procedures.
Your profile:
Bachelor HES in nursing or a title deemed equivalent (recognition by the Croix-Rouge is indispensable)
Good knowledge of the Swiss socio-sanitary field (an asset)
Excellent analytical, synthesis, and communication skills.
Mastery of French (oral and written) and writing skills.
Ease in managing priorities and taking initiative.
Mastery of common IT tools.
Team spirit, sense of service, and ability to work autonomously as well as in interdisciplinary collaboration.
We offer
Schedules mainly during the day (7h00–17h00), with only one evening worked every two weeks until 22h00 and one weekend worked every eight weeks.
Working and remuneration conditions according to the CCT of the Vaudois parapublic sanitary sector.
Quality occupational pension scheme, beyond the legal minimums.
5 weeks of vacation per year, then 6 weeks from 50 years old.
