Dipl. Pflegefrau/Pflegefachmann HF as Deputy Specialist Management Inhouse Spitex 40% ##

The Stiftung Dammweg offers a diversified residential and day structure range and is constantly developing with the goal of offering contemporary support and residential forms for people with disabilities. The residential area has 66 residential places. To ensure high-quality care, we operate an in-house Spitex.

Your tasks

• You support the Specialist Management Inhouse Spitex in the coordination as well as the assessment of the care needs of people with a disability within the Stiftung Dammweg.
• You support at the interfaces between the mobile home doctor practice, the general practitioners, external healthcare professionals, and the consulting pharmacy.
• You use your expertise to commit to professional, person-centered care and ensure the necessary transfer of knowledge. You work occasionally in nursing
• You work on Fridays and on one additional weekday.

Your profile

• Completed training as Pflegefachfrau / Pflegefachmann HF or a completed degree in nursing (Bachelor of Science in Nursing).
• Sound theoretical and practical specialist knowledge in general medicine and nursing, preferably in outpatient care (Spitex).
• Knowledge in needs assessment with the InterRai HC instrument.
• Organizational skills and a goal- and result-oriented way of thinking.
• Interest in working with people with a disability.
