Do you want to support people with dementia in living safely and independently in their familiar home for as long as possible? In the Memory Care Team of Spitex Zürich, you will provide holistic support to customers with cognitive impairments - from needs assessment through to nursing care and counseling relatives. In doing so, you will take on case responsibility and create individual solutions that make daily life easier for those affected and their environment. Your shifts take place during the day - usually between 07:00 and 16:00. Dipl. Pflegefachfrau/-mann mit Fallführung - Demenz 60-100% (a) YOUR TASKS * You will take on case management for people with dementia and manage the nursing process from needs assessment through to planning and the regular evaluation of agreed measures. * You will nurse and care for our customers in their familiar home, promote their existing resources, and support them in maintaining their independence for as long as possible. * You will advise and accompany relatives, coordinate cooperation with doctors and specialist agencies, and serve as an important contact person for all parties involved. * You will work closely with colleagues from various professional groups within the framework of the Skill & Grade Mix and ensure careful and complete nursing documentation. * You will network with internal and external specialists, contribute your knowledge to the team, and support new employees, apprentices, and students during their induction.
