Your tasks ###

• Counseling and support of patients in complex social issues as well as assumption of case management
• Organization of individual and needs-based aftercare and discharge solutions, as well as mediation of material assistance if required and triage to appropriate specialist centers
• Interdisciplinary cooperation with doctors, nursing professionals as well as other occupational groups to ensure coordinated patient care
• Practical training of students of Social Work (BSc) or readiness to complete the corresponding further training

Your profile ###

• Completed studies in Social Work (BSc FH)
• Ideally additional training in the healthcare sector (e.g., FaGe, Pflege HF or FH)
• Training as a practical instructor or readiness to complete this
• High social and counseling competence as well as enjoyment of interdisciplinary cooperation
• Independent, structured and solution-oriented way of working
• Interest in working in the dynamic environment of an acute hospital
