Then don't wait any longer and join us!* We are looking for the CMS of Moudon, a ASSC at 50-80% Mission: • Collaborate on the medical-social support project (accompaniment in the form of aid and care) based on respect for the principle of autonomy of the person. • Participate in monitoring the evolution of the client's situation, in collaboration with the client, their entourage, the interdisciplinary team, and the treating physicians. • Provide clients with services such as basic care, nursing care, household help, and family assistance. Profile: • Have a CFC in ASSC. • Have, if possible, experience in home care. • Be available to work during the day, evening, and weekends. • Be able to work autonomously and in an interdisciplinary manner. • Hold a driver's license and possess a vehicle (possibility of using a professional vehicle).
