Healthcare Professional / Spitex Coordinator (60-100%)

On behalf of our client, a sympathetic, renowned and rapidly growing Spitex organization (PA-model) with a sympathetic corporate culture (workplace Stadt Zürich or Stadt Bern selectable, central locations, parking spaces in front of the house), we are looking for an experienced FaGe for the expansion of the team for coordination and case management in nursing. Healthcare Professional / Spitex Coordinator (60-100%) # In this versatile function, you are part of a collegial, dynamic team (Dipl. Pflegefachperson/FaGe/PA in care) and, as a competent specialist, you take over the support/coaching of the family caregivers (PA), who undertake the care/nursing of the clients on-site. You participate in case management, are on-site for needs assessments and support the PA with regular visits to clients and in their training/instruction. You support case documentation, coordinate billing with the health insurance companies and ensure optimal client care as well as the provision of nursing services. As a communicative personality, you also find the active cooperation and exchange of information with internal departments (deployment planning, Dipl. Pflegefachkollegen) as well as external contact persons (authorities, health insurance companies, caring relatives as well as the care team) easy. Fachperson Gesundheit (FaGe) or an equivalent further training (or are in training for this). Additionally, you have already been able to gain versatile practical experience in coordination in the nursing sector (ideally including Spitex). You enjoy supporting a wide variety of people in everyday life, instructing and accompanying relatives in care, and building a professional relationship based on trust. You work with a high degree of independence, are well-organized and possess great teaching ability as well as a healthy dose of humor.
