We are looking for a day nurse to strengthen its health services. Your mission is to ensure the health care of several living groups. You guarantee the continuity and quality of nursing care, develop prevention and health promotion actions, and work in close interdisciplinary collaboration with the socio-educational staff and network partners, in respect of the institutional concept. Day Nurse (HES) 80% Mission: Health care and continuity of care: - Be the reference person in matters of health for several living groups - Ensure the continuity of preventive, curative, palliative and delegated nursing care - Identify critical situations and respond to them in an adapted manner Prevention and health promotion: - Implement prevention, health promotion and screening actions - Promote hygiene measures and health-friendly behaviors - Inform and sensitize teams and guests to health issues Interdisciplinary work: - Collaborate closely with socio-educational staff in support - Participate in interprofessional meetings and contribute to common reflections - Work with external partners (doctors, hospital, CMS, etc.) Monitoring and quality: - Keep medical records and nursing writings up to date - Develop health syntheses and care projects.
