Job Description ##

• Independent nursing and care of clients at home
• Planning, implementation, and evaluation of the nursing process
• You perform assessments using interRai HC and can take over case management tasks
• Close interprofessional cooperation with doctors, therapists, and relatives
• Taking on case responsibility and nursing coordination
• Participation in quality and development processes
• In the morning in somatic nursing, in the afternoon case management tasks or assessments.
• 1-2 weekends per month. Day service always from 7.00-16.30

Requirement Profile ##

• Completed training as a Dipl. Pflegefachperson HF or FH.
• Professional experience in a relevant specialist area is an advantage.
• Sense of responsibility and high social competence.
• Resilience, flexibility, and teamwork skills.
• Knowledge of IT/PC applications.
• Readiness for flexible working hours and weekend services.
• Very good German skills in speaking and writing.
