As a supplement to our existing specialist responsible for caring relatives, we are looking for an additional Dipl. Pflegefachperson HF/ FH with an additional function as specialist responsible for caring relatives, starting immediately or by arrangement. Your versatile tasks as a nursing specialist are: You are active in direct care and take over the consultation and guidance of customers, relatives and employees in complex care situations. You carry out needs assessments using the InterRAI instrument (RegioSpitex Limmattal covers the costs for the InterRAI further training). As a case manager, you are responsible for the planning and implementation of the care process for your customers. You play an active role in cooperation with the caring, interdisciplinary network and coordinate the corresponding services for your customers. Your additional tasks together with a colleague as specialist responsibility for caring relatives are: You lead, advise, accompany and promote the caring relatives in technical and social situations, conduct regular status meetings, ensure the flow of information and organize needs-based training. You shape the entire personnel process in cooperation with HR - from recruitment to induction to offboarding. You ensure that the caring relatives complete the required SRK course within the specified period, and accompany them in implementing what they have learned in practice. You take over the case management of the customers of caring relatives including the administrative steps, coordinate the deployment planning and ensure quality on site. You bring with you: You have a degree as Dipl. Pflegefachperson HF / FH as well as corresponding professional experience in this field. You have very good oral and written German skills (level C1). You bring experience in the technical accompaniment and leadership of employees as well as strong organizational and coordination skills. You are an open-minded and flexible personality who likes to approach people. You bring good communication skills as well as confident handling of mobile nursing documentation (smartphone).
