For our social counseling service, we are looking for as of 1 December 2026 or by arrangement a

Social Worker FH/HFS (a) 40% ##

Your tasks ###

• Accompanying patients and their social environment through targeted support and counseling processes in cooperation with the treatment teams
• Accompanying the discharge or transition process of patients
• Counseling on health, accident, and social insurance technical questions
• Counseling and organization of spa, rehabilitation, or nursing home stays
• Networking patients with various counseling centers
• Responsibility for the complete documentation of ongoing cases including maintaining statistics
• Participation in interdisciplinary meetings and reports

Your profile ###

• Diploma from a recognized University of Applied Sciences (FH) or Higher Vocational School (HFS) in the field of Social Work
• Empathy as well as a differentiated self- and external perception
• Ability to apply social work methodological knowledge in the areas of mandate clarification, conversation management, situation analysis, and intervention
• Ability to establish a trusting cooperation with patients and to work in a goal- and solution-oriented manner
• Enjoyment of interdisciplinary cooperation
• Confident handling of electronic work tools and file management
• Teamwork skills, resilience, and flexibility
• Interest in professional and health policy issues and developments
• Participation in the creation of concepts, in working groups and projects within the organization
