Your tasks

• Global care and support of residents in accordance with our care concept and the care process
• Taking charge of tasks as a reference person in care
• Responsibility and management of the care process of assigned residents, including the assessment of care needs RAI/BESA
• Electronic documentation of care via CareCoach
• Planning and support of medical visits
• Taking charge of daily responsibility with coordination of the day team
• Conducting case meetings and care reports
• Taking charge of tasks as a thematic referent in a specialized field and participating in the quality network of care within the group
• Support and advice to employees regarding questions related to RAI LTCF and the care process
• Detection of changes in care and support needs as well as rapid adaptation of the corresponding planning
• Participation in case meetings, care visits, and interprofessional meetings
• Promotion of person-centered and resource-oriented care
• Active participation in the guarantee and continuous development of the quality of our care

Your profile

• Completed training as a Registered Nurse HF/FH/DNII or equivalent training recognized by the Swiss Red Cross
• Completed training as a Health Specialist EFZ
• Professional experience in long-term care desired
• In-depth practical experience with RAI LTCF
• Professional experience in Spitex or internal Spitex desired
• Very good knowledge of the care process and professional care documentation
• Mastery of performing and conducting assessments
• Great sense of quality and responsibility
• Autonomous, structured, and reliable work
• Joy in transmitting knowledge and supporting colleagues on a daily basis
• Respectful and valuing attitude towards our residents and employees
• Good command of German orally and in writing as well as ease with digital documentation systems
