What awaits you:

• You independently plan, organize, and create needs assessments and care plans for the clients assigned to you.
• You plan, organize, and accompany the assigned clients according to the mandate and ensure the associated administrative tasks - from assessment to service recording and evaluation to progress documentation.
• You work throughout the entire Canton Schwyz and accompany our clients in their respective living environments.
• You maintain a goal-oriented and appreciative exchange with important internal and external partners, especially with employers, relatives, legal representatives, doctors, and other professional support services.
• You ensure the up-to-dateness of the administrative master data and waivers of confidentiality for the clients assigned to you.
• You reliably report administrative changes to the Central Services.
• You actively participate in team meetings as well as team and case supervisions and contribute your professional perspective.

What you bring with you:

• Completed training as a dipl. Pflegefachperson HF/FH with the qualification to take over case management including needs assessments, or the willingness to acquire this
• Several years of professional experience in psychiatric nursing and needs assessment
• Several years of experience in accompanying adults with mental illnesses
• High social competence and pronounced communicative skills
• Independent, structured, and reliable way of working
• Pronounced planning and organizational skills
• Flexibility and willingness to organize the work week independently and according to needs
• Valid driver's license category B and own vehicle mandatory
• Confident German in speaking and writing
• Confident handling of MS Office, smartphone, and digital work tools
• Willingness to work throughout the entire Canton Schwyz
