On 1. January 2025, careköniz was created from the merger of SPITEX Region Köniz and logisplus and today employs around 370 employees. Become part of our committed team and actively shape the future of care! These are your tasks: You conduct professional assessment and consultation interviews with the various stakeholders. You inform about the tariffs and the basics of financing the residential stay. You plan the admission to the home and advise and support all those involved with expertise. You organize and coordinate internal and external transfers or discharges to home. You inform the nursing staff about the collected assessment information. You work interdisciplinarily with internal and external specialists and ensure a professional exchange of information. You support relatives in filling out applications for disability allowances. What we wish for: You have a nursing diploma education as well as corresponding professional experience in long-term care, ideally also in the acute care sector. With your empathy and solution-oriented way of working, you create sustainable relationships. You quickly recognize the problem areas of the stakeholders and use your broad knowledge and extensive expertise during the needs assessment. You have very good communication skills, speak and understand German very well; French and English skills are an advantage. You are a team-oriented and responsible personality who also enjoys working independently. You have a flair for administrative activities and are proficient in using software programs.
