Missions

The POOL-EMRAD (Équipe Mobile de Retour à Domicile) has the following missions:

• Take charge of the patient base at home during the first days after hospital discharge
• Ensure nursing replacements and coverage of extended hours within the CMS de l’Est vaudois.
• Ensure nursing care in the evening and 2 weekends per month for the CMS de l’Est vaudois.
• Coordinate care during weekends and public holidays (For people having the necessary qualities for this support and guide function for the field).
• After a period of integration and specific training regarding the diversity of situations and the population of the CMS, you will be autonomous in the management of nursing care.
• Accompanied and surrounded by an expert multidisciplinary team, you perform medico-technical care and ensure the follow-up of our beneficiaries.
• You develop a care project with the beneficiary, taking into account their priority needs, their resources, those of their entourage and the Équipe Mobile de Retour à Domicile.
• You mobilize internal and external resources, coordinate the care and ensure follow-up of the situation, including through the computer update of files and your active participation in colloquiums and team meetings.
• Your activities are carried out in close collaboration with the multidisciplinary team, the medico-social network, hospital and/or on-call doctors, as well as partner medico-social institutions.
• By prioritizing a global approach to the person, you ensure prevention and health promotion among our beneficiaries and their entourage.

Location

Offices based in Rennaz, travel in the districts Riviera-Pays-d'Enhaut and d’Aigle

Your profile

• Bachelor's in nursing in general care, level 2 or HES. In a desire for complementarity of skills, young graduates or experienced persons are welcome.
• Sense of responsibility and professional ethics.
• Appreciate working in an interdisciplinary team and collaboration with different actors of the health network.
• Great sense of relationship, communication and negotiation.
• Proactivity, reactivity and solution orientation.
• Versatility, flexibility and adaptability.
• Comfortable with IT tools (management of the patient file, care planning, monitoring of quality indicators).
• For foreign diplomas, a Red Cross recognition is required or the process must be underway.
• Driver's license and vehicle indispensable.
