Spitex Coordinator / Registered Nurse HF/FH (60-100%)

On direct behalf of our client, a sympathetic, renowned, and strongly growing Spitex organization (PA-model) with a sympathetic corporate culture, we are looking for an experienced Dipl. Registered Nurse HF/FH for coordination and case management (without nursing activities) to expand the team. You will strengthen (selectable) the team in the city of Bern or in the city of Zurich (workplace selectable, both central locations, parking spaces available). Spitex Coordinator / Registered Nurse HF/FH (60-100%) # In this versatile role, you are part of a collegial, dynamic team and, as a competent specialist, you will take over case management as well as care planning for the care including preventive support of clients at home. You plan the care including needs assessment, target specifications, and deployment planning for the FaGes and caring relatives (PA), who care for the patients on-site. You take over the case management and coordination with the health insurance companies, provide advisory support to the teams, and assist in the consultation of clients (by telephone, occasionally also on-site - e.g., during assessments). Together with your team, you ensure the optimal provision of nursing services and client care. As a communicative personality, you also find active cooperation and the exchange of information with various contact persons (management, FaGes, care team, external bodies such as health insurance companies or authorities) easy. To be successful in this exciting job, you have completed the diploma as a Registered Nurse HF/FH or an equivalent further education (or are in training for it). Additionally, you have already been able to gain versatile practical experience in case management and coordination in the nursing sector (ideally including Spitex). You enjoy communicating with a wide variety of contact persons, accompanying nursing cases with your teams, and building a professional relationship based on trust. You work with a high degree of independence, are well-organized, and possess great capacity for introduction as well as a healthy dose of humor.
