For our sympathetic and dynamic team, we are looking for a Certified Nursing Professional HF/FH (m/f/d), 60-90% in the dementia team by arrangement. Your field of activity: Your field of activity. You are responsible for optimal care and advice for our clients with a focus on dementia. With case management, you take responsibility for all steps in the nursing process of your clients. You take on coordination tasks with other involved services and participate actively in the cooperation with our interprofessional team. How you convince us: Completed training as a certified nursing professional (DNII / HF / FH). Experience in the nursing and care of people with dementia. Empathic, innovative, flexible, and cheerful personality with strong social, communication, and self-competencies. Flexibility and resilience. Driver's license Cat. B.
