For our specialized specialist team, we are looking for a motivated nursing specialist HF/FH with a focus on dementia (40 - 80%) Your tasks: You are responsible for the qualified and resource-oriented care and support of our customers and their relatives. You accompany the entire care process and carry out needs assessments as well as round-table discussions. You coordinate cooperation with other specialists and ensure that relatives and external services are optimally involved.
