Surrounded by vineyards, with an incredible view of the mountains and the lake, Hôpital de Morges welcomes you in a warm and dynamic setting.* We are looking for Hôpital de Morges, a: Care Coordinator for chronic patients and complex situations F/H at 80% to 100% YOUR MISSION In collaboration with family doctors, you ensure the follow-up and coordination of patients with chronic diseases or complex medico-psycho-social situations in an autonomous manner. Thanks to your organizational skills and initiative, you anticipate patients' needs and actively contribute to improving their care pathway. You play a central role in organizing this pathway to ensure continuity, quality, and economic efficiency of services. WE OFFER YOU • 5 weeks of annual vacation, 6 weeks from the age of 50. YOU POSSESS • A recognized CRS nurse diploma or a federal certificate of Coordinator in ambulatory medicine clinical orientation. • The ability to juggle multiple interlocutors and situations while maintaining a patient-centered approach. • An excellent sense of organization allowing effective prioritization and management of a large patient portfolio. • The ability to work autonomously while implementing an interprofessional approach. • Good mastery of computer tools and clinical documentation. • Good knowledge of the healthcare network and medico-social actors. • Excellent relational and communication skills.   YOUR OBJECTIVES • Ensure, in an autonomous and collaborative manner with family doctors, the coordination and support of patients with chronic diseases or complex medico-psycho-social situations. • Evaluate patients' needs and identify medical, psychosocial, organizational, and economic challenges. • Coordinate interventions between different professionals involved (doctors, nurses, therapists, social services, caregivers, etc.). • Ensure regular patient follow-up, anticipate risk situations, and facilitate access to adapted resources and services. • Develop a care partnership with patients to strengthen their autonomy, active participation in decisions, and adherence to treatments and recommendations. • Participate in the continuous improvement of processes, care pathway quality, and ensure rigorous documentation of interventions.   WE ARE • An institution comprising acute care and rehabilitation facilities, a private clinic, EMS, medical centers, and permanent facilities located in the La Côte region, as well as the 1st clinical hypnosis school dedicated to the care environment in French-speaking Switzerland. • A care network offering many opportunities for internal mobility according to the aspirations and development projects of employees. • More than 2000 employees taking care of patients and residents in the region, sharing values of Responsibility, Proximity, Humanity, Competence, in an agile environment favoring project implementation. • A training company committed to training the next generation, with around 80 apprentices in 16 different trades, 200 HES-ES interns, and nearly 300 doctors in training.
