In close collaboration with the multidisciplinary team and the situation referents, you will play a central role in the implementation and evaluation of care objectives. Your main missions: • Perform medico-delegated care, basic care, and household assistance in compliance with prescriptions and individualized needs. • Observe, evaluate, and transmit changes in the health status of beneficiaries in order to adapt the care. • Support relatives and families, and contribute to interdisciplinary coordination. • Actively participate in the continuous improvement of care quality and the sharing of practices within the team. • Contribute to maintaining home care and the promotion of community health Location: CMS de Rennaz Profile: • CFC d-ASSC - Assistant in nursing and community health • Sense of responsibility and professional ethics • High sense of relationship, communication, and negotiation • Autonomy, versatility, flexibility, and adaptability • Proactivity, reactivity, and solution orientation • Good capacity for analysis and evaluation, as well as for collaborating in networks and interdisciplinarity • You are autonomous, dynamic, and enjoy working in a team • Comfortable with IT tools (patient file management, care planning, monitoring of quality indicators). • Driver's license and vehicle indispensable • For foreign diplomas, a Red Cross recognition is required or in the process of being obtained.
