Dipl. Pflegefachpersonen HF/FH

We are looking for Dipl. Fachpersonen HF/FH with case management - Acute and Chroin Care 60-90% for one of our Spitex customers.

YOUR TASKS:

• Resource- and goal-oriented nursing and care
• Advice and support for relatives
• Case management through planning, controlling and evaluation of the nursing process
• Instruction, coaching and advice of nursing staff of other competence levels
• Responsibility for careful and complete management of nursing documentation
• Induction and support of new employees, apprentices and students
• Interdisciplinary cooperation with employees, specialist agencies and doctors

REQUIREMENTS:

• Completed nursing training at tertiary level
• Friendly and appreciative demeanor as well as physical and mental resilience
• Enjoyment of change and of interprofessional cooperation in the team
• High sense of responsibility, independence and flexibility
• Good knowledge of the German language in speaking and writing, at least level B2
• Driver's license Cat. B or willingness to travel by electric bicycle
