Social Worker FH or Social Pedagogue HF/FH (80-100%)

The Stiftung Puureheimet Brotchorb offers living and working space to adults with mental illnesses and/or substance dependency. On our biodynamically managed farm, 20 residential places are available in a supportive community. The residential offer is supplemented by work and daily structure places in the areas of agriculture, garden, maintenance, and kitchen/housekeeping. Social Worker FH or Social Pedagogue HF/FH (80-100%) Your tasks • Daily responsibility: You will take on day-to-day responsibility in the living area on a rotational basis. In doing so, you coordinate the ongoing residential operations and are the contact person for the concerns of the clients. • Case management: You will accompany four to six clients in a case-managing capacity in the areas of living, everyday life, and leisure, and conduct the corresponding progress meetings. • Case documentation and IBB: You will maintain case documentation in a professionally sound and traceable manner, maintain electronic client files, write professional reports, and collect and document individual support needs (IBB). • Interdisciplinary cooperation: You will coordinate case-related cooperation with referring agencies, legal guardians, medical and therapeutic professionals, and other involved specialist agencies. • Intake: You are responsible for the intake process from the initial telephone contact through the interview and admission assessment to entry. • Medication administration: You administer prescribed medications in a controlled manner and document the administration according to the applicable guidelines. Working hours and on-call duty • Presence times: Your mandatory presence times include the days with daily responsibility, a weekly late shift from 18:00 to 20:00, as well as a monthly weekend service on Saturday from 10:00 to 14:00 and on Sunday from 16:00 to 20:00. You will flexibly coordinate the remaining working time within the support team. • Night on-call: Following the late and weekend shifts, you take over the telephone on-call service and are reachable until 08:00 the following day. As a rule, occurring situations can be clarified or coordinated by telephone. If necessary, an on-site deployment is required within 60 minutes. What you bring with you • Education: You have a degree in Social Pedagogy HF/FH, Social Work FH, or an equivalent qualification. • Professional experience and attitude: Experience in accompanying adults with mental illnesses and/or substance dependency is an advantage. You work in a resource-oriented, appreciative, and equal-level manner. • Self-responsibility: You work in a self-organized manner, set priorities, and make professionally justified decisions within your area of responsibility. Even in demanding situations, you maintain an overview and remain capable of action. • Cooperation and communication: You communicate openly and transparently, deal constructively with feedback, and contribute overarching issues to the team. You support decisions made together. • Digital and administrative competence: You have a confident written expression and use digital work tools routinely. Commercial knowledge is an advantage. • Driver's license: A category B driver's license is required.
