The Universitätsklinik für Diabetologie, Endokrinologie, Ernährungsmedizin und Metabolismus (UDEM) cares for patients with complex metabolic diseases at the highest university level. To strengthen our interprofessional team, we are looking for a committed and motivated personality as a Diabetesberater:in HFP/DAS/HöFa1

What awaits you ##

• Counseling and training of outpatient and inpatient patients with diabetes mellitus (Type 1, Type 2, and gestational diabetes)
• Support and instruction in insulin therapies as well as modern diabetes technologies (CGM, insulin pumps, and automated insulin delivery systems)
• Promotion of self-management and health literacy of patients and relatives
• Implementation of individual and group training sessions
• Interprofessional collaboration with physicians, nutritionists, nursing professionals, and other specialist disciplines
• Documentation in the electronic patient dossier
• Participation in the further development of diabetes counseling

What you bring with you ##

• Diploma as a Pflegefachperson HF/FH or an equivalently recognized training
• Completed further training as a Diabetesberater:in HFP, DAS or comparable qualification (Master's degree)
• Experience in caring for people with diabetes, ideally in an acute hospital or outpatient setting
• Enjoyment of patient-centered, evidence-based counseling
• High social, communication, and counseling competence
• Independent, structured, and responsible way of working
• Interest in innovation, digitalization, and the further development of modern diabetes care
