For our team with a focus on Consiliar- Geriatric and Neuropsychiatry (KAN), we are looking for you as a Social Worker FH ## Windisch 50-50% The cross-sectional area of Nursing, Education and Social Services consists of the following: Competence Center Education, personnel pool flex26, nursing development, de-escalation management, social services and central registration. The Competence Center Education supports staff with and during initial, advanced and continuing education and thus makes an important contribution to ensuring a professional and high-quality level of nursing. Nursing quality is also a central concern of nursing development, which leads cross-ward projects on the basis of evidence-based knowledge together with the specialist managers. Thanks to our own personnel pool flex26, we succeed in compensating for short-term vacancies/absences quickly and professionally and compensating with PDAG-internal nursing staff. De-escalation management supports the treatment team in the competence of de-escalating milieu design, advises and supports employees before and after threatening situations. It develops interdisciplinary training concepts and conducts continuing education on this topic. The social service is an important component of the interprofessional treatment offering. It supports and promotes patients in social and professional integration with the aim of regaining their independence in everyday life. The central registration is the first point of contact for external referrers, helps with questions about our services, receives referrals and triages them. Profile • Bachelor of Science / of Arts from a Fachhochschule for Social Work • Professional experience in working with older people or a comparable environment • High appearance competence, service orientation, initiative, flexibility, independence and resilience • Interested and empathetic personality with joy and motivation in networked working • Your strengths include a positive view of humanity, social competence, solution orientation as well as a healthy amount of humor and pragmatism • You are a team player with a high sense of responsibility and quality awareness and are used to setting priorities and working independently • You place the dignity, self-determination and social participation of older people at the center of your work and can recognize the associated opportunities and limits and deal with them professionally. Tasks • You advise patients and their relatives regarding suitable forms of housing and daily structure as well as in social insurance law, financial and adult protection law issues. In doing so, you are in close exchange with the specialists involved. • You promptly record the needs of newly admitted patients and their relatives and jointly seek suitable and sustainable follow-up solutions for discharge from the clinic.
