Infirmier RAI - HC (M/F)

Activity rate between 80%

Permanent contract for an indefinite period

Your mission

• Perform evaluations and re-evaluations of all client situations at their home, using the RAI-HC (Resident Assessment Instrument - Home Care) method
• Evaluate situations and identify needs
• Plan, coordinate and provide care and ensure follow-up
• Advise and inform the people being cared for and their entourage
• Contribute to the maintenance of health and the prevention of illness through an interdisciplinary approach
• Strengthen the autonomy and adaptation capacities of the person
• Develop the quality of care and adapt services according to new knowledge

Your profile and your skills

• Bachelor in nursing or title deemed equivalent (foreign diploma recognized by the CRS)
• RAI-HC evaluator training an asset
• Proven experience of 2 years in the medico-social field, in interdisciplinary collaboration and networks
• Sense of responsibility, ability to organize autonomously and to set priorities
• Ability to collaborate in networks and multidisciplinarity
