Spitex Coordinator / Registered Nurse HF/FH (60-100%) / Permanent Introduction On behalf of our client, a sympathetic, renowned, and rapidly growing Spitex organization (PA-model) with a sympathetic corporate culture (headquarters in Bern and Zurich, among others, central location, parking spaces in front of the building), we are looking for an experienced Registered Nurse HF/FH for the expansion of the team at the Zurich location for team coordination and case management. Nature of the work In this versatile role, you will be part of a collegial, dynamic team and, as a competent specialist, you will take over case management as well as care planning for care including preventive support for clients at home. You plan the care including needs assessment, target specifications, and deployment planning for the FaGes and caring relatives (PA), who care for the patients on-site. You take over case management and coordination with the health insurance companies, provide advisory support to the teams, and assist in advising clients (by telephone, occasionally also on-site - e.g., during assessments). Together with your team, you ensure optimal provision of nursing services and client care. As a communicative personality, you also find active cooperation and the exchange of information with various contact persons (management, FaGes, care team, external bodies such as health insurance companies or authorities) easy. Requirement profile To be successful in this exciting job, you have completed training as a Registered Nurse HF/FH or an equivalent further education (or are currently in training for it). Additionally, you have already been able to gain versatile practical experience in case management and coordination in the nursing sector (ideally including Spitex). You enjoy communicating with a wide variety of contact persons, accompanying nursing cases with your teams, and building a professional relationship based on trust. You work with a high degree of independence, are well-organized, and possess great capacity for introduction as well as a healthy dose of humor.
