In this versatile function, you combine Social Work with the co-management of the Disposition & Care and Case Management area. You take on leadership responsibility, develop the area further and simultaneously contribute your specialist competence in the counseling and support of residents, patients and relatives. In doing so, you work closely with various professional groups and external agencies and are active at our locations in Bassersdorf and Embrach.

Your tasks

• Direct leadership of approx. eight employees
• Counseling and support of residents, patients and relatives with questions regarding social insurance, nursing care financing and living arrangements
• Counseling and processing of inquiries regarding care places and service offers
• Case management in complex care situations in the long-term sector
• Support in case management in rehabilitative acute and transitional care
• Analysis and further development of work processes as well as participation in quality assurance

Your profile

• Completed studies in Social Work
• Leadership and project management experience
• Professional experience in the healthcare sector
• Good knowledge in the area of social insurance and nursing care financing
• Independent, structured and responsible way of working
• Strong communication and teamwork skills
• Enjoyment of interprofessional cooperation and of taking on leadership responsibility
