In the Bärn-Netz, general practitioner practices, specialists, hospitals, therapeutic services, nursing, and Spitex services work more closely together to better accompany patients along their health pathway. The goal is coordinated, local, and high-quality care, with strong primary care as the central anchor.

With VIVA, an alternative basic insurance model is emerging within this network, focusing on prevention, health coordination, Chronic Care Management, and personal accompaniment. Health coordination supports patients and general practitioners in everyday life, creates orientation within the care system, and helps to avoid unnecessary redundancies, emergency consultations, and hospitalizations.

To strengthen our team, we are looking for a committed APN Nurse Practitioner. You will work closely with physicians, therapists, the nursing team, as well as an already integrated APN, and will actively further develop the APN role in primary general practice as well as for the new basic insurance model VIVA.

Job Description

• Conducting anamneses and clinical examinations as well as holistic assessment of nursing, medical, and social needs
• Clinical Reasoning, initiating supplementary diagnostic investigations, and formulating initial medical and nursing (suspected) diagnoses, if necessary in consultation with the responsible physician
• Planning and implementing suitable nursing and medical measures based on the findings collected
• Medication management including prescription, review, and adjustment within the framework of applicable competencies, as well as training patients in the safe handling of medications
• Conducting consultations, advice sessions, and training in prevention, health promotion, and self-management
• Accompanying patients and relatives with chronic illnesses as well as promoting adherence and health literacy
• Initial assessment of acute situations, assessment of urgency, and initiation of necessary measures or early involvement of the medical staff
• Interprofessional cooperation with internal and external specialists as well as active co-design of care processes
• Recognizing ethical issues and participating in joint, patient-oriented decisions
• Independent action within the defined area of responsibility and competence, as well as continuous reflection and further development of one's own role

Qualifications

• Completed Master of Science in Nursing (MScN), ideally with a focus on Nurse Practitioner
• Ideally professional experience in primary general practice and/or health coordination
• Very good knowledge of medical terminology and proficient German skills in speaking and writing
• Good Microsoft Office skills, especially Word, Excel, and Outlook; experience with KIS or PIS software is an advantage
• Independent, responsible, and reliable way of working with high quality standards
• Initiative, resilience, capacity for reflection, and strong organizational skills
• Networked thinking and a solution-oriented way of working
• Enjoyment of interprofessional cooperation as well as secure, empathetic, and service-oriented communication
• French and English skills are an advantage
