Advanced Practice Nurse (APN) / Clinical Specialist 80-100%

To strengthen our physician and Nurse Clinical Practitioner (NCP) team in the Department of Internal Medicine, we are looking for a committed personality to start immediately or by arrangement. In this role, you will take on a central role in the care of our patients and work closely with the medical service as well as other professional groups. You will accompany patients throughout the entire diagnostic and treatment continuum and also contribute your clinical expertise to innovative care models. #### Advanced Practice Nurse (APN) / Clinical Specialist 80-100% # Your tasks #### • You support the medical teams on the inpatient wards of the Department of Internal Medicine within the interprofessional team. • Together with or delegated by senior physicians, you care for patients throughout the entire diagnostic and treatment continuum. • You take on clinical, organizational, and administrative tasks in innovation projects such as Hospital at Home and Geriatric Co-Management. • You participate in tasks within the framework of clinical accompanying research. • You coordinate care transition in the hospital as well as the transition from the inpatient to the outpatient sector. • You carry out pre- and post-inpatient assessments and assess within the interprofessional team whether further measures are necessary. Your profile #### • You have a degree as MScN, BScN or MAS/DAS Physician Associate. • Ideally, you already bring professional experience as a Clinical Specialist or Advanced Practice Nurse (APN). • You possess a driver's license of category B.
