The mission of the Toises – Centre de psychiatrie et psychothérapie, is to offer high-quality specialized outpatient care to children, adolescents, and adults of all ages with mental health disorders, and to do so in an optimal, personalized, and adapted manner to each individual's needs. The multidisciplinary team, focused on innovative technologies, works in close collaboration to place the patient at the center of care. • Social Assistant (m/f) Immediate start Position 80-100% - CDI Role description: • As part of the development of our activities, we are currently recruiting two social assistants: one position in Lausanne and one position in Yverdon. • Conduct social situation assessments, follow-up, and social support for patients at our center, taking into account the psychiatric dimension - Work within a multidisciplinary team consisting of psychiatrists, psychologists, nurses, neuropsychologists with the mission of ensuring integrated psychiatric and psychotherapeutic follow-up • Coordinate the patient's social care in collaboration with the different stakeholders at the center and within the network (CMS, CSR, SCTP, DGEJ, etc.) • Lead educational and support interventions in concrete approaches (economic, administrative, professional, relational, legal, housing search assistance, search for a place in an institution) • Assume the role of administrative referent, pending the establishment of adult protection measures by the Justice of the Peace, and collaborate with the patient to avoid loss of social benefits Your profile: • Bachelor HES in Social Work, social service orientation or equivalent title • You have professional experience that allows you to work in perfect autonomy.
