Healthcare Specialist as Spitex Coordinator (60-100%)

On direct behalf of our client, a sympathetic, renowned and strongly growing Spitex organization (PA-model) with a sympathetic corporate culture (headquarters in, among others, Stadt Bern and Zürich, central location, parking spaces in front of the building), we are looking for an experienced FaGe for coordination and case management to expand the team for the new headquarters in Bern. Healthcare Specialist as Spitex Coordinator (60-100%) # In this versatile role, you are part of a collegial, dynamic team (Dipl. Pflegefachperson/FaGe/PA in care) and, as a competent specialist, you take over the support/coaching of the caring relatives (PA), who handle the care/nursing of the clients on-site. You participate in case management, conduct needs assessments on-site, and support the PA with regular visits to clients and in their training/instruction. You support case documentation, coordinate billing with the health insurance companies, and ensure optimal client care as well as the provision of nursing services. As a communicative personality, you also find active cooperation and the exchange of information with internal departments (deployment planning, Dipl. Pflegefachkollegen) as well as external contact persons (authorities, health insurance companies, caring relatives as well as the care team) easy. In addition, you have already been able to gain versatile practical experience in coordination within the nursing sector (ideally including Spitex). You enjoy supporting a wide variety of people in everyday life, instructing and accompanying relatives in care, and building a professional relationship based on trust. You work with a high degree of independence, are well-organized, and possess great capacity for instruction as well as a healthy dose of humor.
