For our specialized dementia team, we are looking for a committed, licensed nursing specialist who accompanies people with dementia in their familiar environment in a competent and empathetic manner. Your Mission • You care for and nurse people with dementia at home - individually, respectfully and professionally • You take over case management and work closely with relatives, general practitioners and other specialist offices • You recognize changes at an early stage and design suitable support services with • You are happy to share your knowledge - e.g. with colleagues or learners in the team • You bring in your expertise and help to further develop our dementia-specific services Your Backpack • Diploma as a nursing specialist HF/FH (or equivalent SRK recognition) • CAS Person-centered care for people with dementia or further training as a dementia coach is an advantage • Joy and experience in dealing with people with dementia - or great interest in deepening yourself in this area • Independent, reliable work and a good sense of people • You are mobile (driver's license category B), like to be on the move and appreciate the variety in Spitex work Our Promise • A committed, interdisciplinary team that works according to the current state of knowledge • High independence in daily planning • Regular further training and support in your development • Attractive employment conditions and modern working tools ?
