Healthcare Professional as Spitex Coordinator (60-100%)

On behalf of our client, a sympathetic, renowned and rapidly growing Spitex organization (PA-model) with a sympathetic corporate culture (headquarters in, among others, the city of Bern and Zurich, central location, parking spaces in front of the building), we are looking for an experienced FaGe for coordination and case management to expand the team for the new headquarters in Bern. Healthcare Professional as Spitex Coordinator (60-100%) # In this versatile role, you are part of a collegial, dynamic team (Dipl. Pflegefachperson/FaGe/PA in care) and, as a competent specialist, you take on the support/coaching of the caring relatives (PA), who perform the care/nursing of the clients on-site. You participate in case management, conduct on-site assessments of needs, and support the PA with regular visits to clients and in their training/instruction. You support case documentation, coordinate billing with health insurance companies, and ensure optimal client care as well as the provision of nursing services. As a communicative personality, you also find the active cooperation and exchange of information with internal departments (scheduling, Dipl. Pflegefachkollegen) as well as external contact persons (authorities, health insurance companies, caring relatives as well as the care team) easy. Furthermore, you have already gained versatile practical experience in coordination in the nursing sector (ideally including Spitex). You enjoy supporting a wide variety of people in everyday life, instructing and accompanying relatives in care, and building a professional relationship based on trust. You work with a high degree of independence, are well-organized, and possess great teaching ability as well as a healthy dose of humor.
