Healthcare Professional / Spitex Coordinator (60-100%)

On behalf of our client, a sympathetic, renowned and rapidly growing Spitex organization (PA-model) with a sympathetic corporate culture (work location City of Zurich or City of Bern selectable, central locations, parking spaces in front of the building), we are looking for an experienced FaGe for coordination and case management in nursing to expand the team. Healthcare Professional / Spitex Coordinator (60-100%) # In this versatile role, you are part of a collegial, dynamic team (Dipl. nursing professional/FaGe/PA in care) and, as a competent specialist, you take on the accompaniment/coaching of the caring relatives (PA), who undertake the care/nursing of the clients on-site. You participate in case management, are present on-site for needs assessments, and support the PA with regular visits to clients and in their training/instruction. You support case documentation, coordinate billing with health insurance companies, and ensure optimal client care as well as the provision of nursing services. As a communicative personality, you also find active cooperation and the exchange of information with internal departments (scheduling, Dipl. nursing colleagues) as well as external contact persons (authorities, health insurance companies, caring relatives as well as the care team) easy. To be successful in this exciting job, you have completed further training as a dipl. healthcare professional (FaGe) or an equivalent further training (or are in training for it). Additionally, you have already been able to gain versatile practical experience in coordination in the nursing sector (ideally including Spitex). You enjoy supporting a wide variety of people in everyday life, instructing and accompanying relatives in nursing, and building a professional relationship based on trust. You work with a high degree of independence, are well-organized, and possess great capacity for instruction as well as a healthy dose of humor.
