Le Réseau Santé la Côte (RSLC) is one of the 4 networks of care in Vaud according to the Loi cantonale sur les Réseaux de Soins (LRS). It brings together, at the regional level, healthcare providers (hospitals, EMS, CMS, doctors, etc.) and municipalities. The mission of the network is to improve, in close collaboration with its members, the coordination of care and the orientation of users within the healthcare system. The RSLC manages the Bureau Régional d'Information et d'Orientation (BRIO) whose main missions are: the orientation of people losing autonomy upon leaving the hospital, the management and evaluation of requests for accommodation in EMS, and information on medico-social benefits to the population and professionals. To complete its social offering to its partners « cabinets médicaux », Le Réseau Santé la Côte is looking for Un\[-e assistant\[-e social\[-e BRIO à 50 % en CDI « cabinets médicaux ». Within the BRIO team, in collaboration with the social workers dedicated to EMS accommodation situations and hospital settings, you offer your expertise to clarify the social situations that arise within medical practices. Your approach, your experience, your ability to create links and to address situations to the appropriate services, allow vulnerable people to clarify their social situation and to be accompanied during their care pathway. Your main activities, with the RSLC medical practices: At the request of the doctor, ensure the processing (launching procedures and/or referral to other professionals) of social and insurance-related situations that must be addressed for adult patients whose benefit from care is compromised by social and/or financial difficulties (housing, lack of insurance, lack of specific knowledge and/or support such as a curator, for example). To be a resource person for social questions for medical practices. Your profile: \\- Diploma as a social assistant (Bachelor of Arts HES-SO in social work) or training deemed equivalent. \\- CAS (in mental health and social psychiatry, interprofessional in addictions, or in Social Management) -- an asset. \\- At least 2 years of experience in the health sector (hospital, extra-hospital and/or in EMS and/or medico-social institution experience), as well as in network work. \\- Very good knowledge of social insurance, the Vaud medico-social system and procedures in relation to the Justice de Paix. \\- Writing skills and mastery of common IT tools. \\- Proven ability to work in a multidisciplinary team and in a network. \\- Proactivity in the field of competence. \\- Negotiation skills, listening and empathy. \\- Vehicle and driver's license necessary.
