As a Care Manager:in, you take on a central role in the discharge process and ensure a smooth transition to post-stationary care with organizational talent and empathy.

• * You conduct initial interviews with patients and relatives to plan the discharge and coordinate the structured exchange of information with external service providers
• You ensure the complete and timely transmission of relevant documents to subsequent treating institutions and document the discharge process continuously in the clinic information system
• You work closely with doctors, nursing staff, social services, and other specialist disciplines, shaping a smooth interprofessional process
• * A diploma as a nursing specialist HF/FH and professional experience
• A way of working that is characterized by high reliability, clear, appreciative, and empathetic communication
• Pronounced organizational strength and the ability to maintain an overview even in complex situations
• Enjoyment of interdisciplinary collaboration and high customer orientation
