Mission

• Ensure care and management adapted to people of all ages, in various contexts, in collaboration with their surroundings.
• Actively participate in a multidisciplinary manner in the organization and follow-up of the care project defined with the client.

Profile

• Bachelor HES-SO in Nursing or equivalent or foreign diploma recognized by the Swiss Red Cross
• Skills in managing a patient portfolio based on continuous assessment and monitoring of care objectives
• Knowledge of the home care field, an asset
• Ability to collaborate within a multidisciplinary team
• Listening skills and helping relationship skills
• Ability to be proactive and to position oneself as a partner in networked work
• Ability to adapt to change, to function autonomously and to manage complex and/or crisis situations
• Available, flexible and good capacity for adaptation
• Driver's license indispensable
