Coordinator Spitex / Nursing Specialist HF/FH (60-100%)

On behalf of our client, a sympathetic, renowned and strongly growing Spitex organization (PA model) with a sympathetic company culture, we are looking for an experienced Dipl. nursing specialist HF/FH for coordination and case management (without nursing activities). You can choose to join the team in the city of Bern or in the city of Zürich (workplace can be chosen, both are central locations, parking spaces available). Coordinator Spitex / Nursing Specialist HF/FH (60-100%)# In this versatile function, you are part of a collegial, dynamic team and, as a competent specialist, take over case management and nursing planning for care, including preventive support for clients at home. You plan nursing care, including needs assessment, target specifications and deployment planning for FaGes and caring relatives (PA), who care for patients on site. You take over case management and coordination with health insurance companies, advise the teams and support them in client consulting (by phone, occasionally also on site - e.g. for site determinations). Together with your team, you ensure optimal provision of nursing services and client care. As a communicative person, you also find it easy to cooperate actively and exchange information with various contact persons (management, FaGes, care team, external offices such as health insurance companies or authorities). To be successful in this exciting job, you have completed a dipl. nursing specialist HF/FH or equivalent further training (or are in training). Additionally, you have already gained versatile practice in case management and coordination in the nursing sector (ideally including Spitex). You enjoy communicating with different contact persons, accompanying nursing cases with your teams and building a professional relationship based on trust. You work with a high degree of independence, are well organized and have a great ability to introduce yourself, as well as a healthy dose of humor.
