Healthcare Professional as Spitex Coordinator (60-100%) / Permanent Introduction On behalf of our client, a sympathetic, renowned, and rapidly growing Spitex organization (PA model) with a sympathetic corporate culture (headquarters in Bern and Zürich among others, central location, parking spaces in front of the building), we are looking for an experienced FaGe for coordination and case management to expand the team for the new headquarters in Bern. Nature of work In this versatile function, you are part of a collegial, dynamic team (Dipl. Pflegefachperson/FaGe/PA in care) and, as a competent specialist, you undertake the support/coaching of the caring relatives (PA), who undertake the care/nursing of the clients on-site. You participate in case management, conduct needs assessments on-site, and support the PA with regular visits to clients and in their training/instruction. You support case documentation, coordinate billing with health insurance companies, and ensure optimal client care as well as the provision of nursing services. As a communicative personality, you also find active cooperation and information exchange with internal departments (deployment planning, Dipl. Pflegefachkollegen) as well as external contact persons (authorities, health insurance companies, caring relatives as well as the care team) easy. Requirement profile To be successful in this exciting job, you have completed training as a Healthcare Professional (FaGe) or an equivalent education and/or further training (or are currently in training for it). Additionally, you have already been able to gain versatile practical experience in coordination within the nursing sector (ideally including Spitex). You enjoy supporting a wide variety of people in everyday life, instructing and accompanying relatives in care, and building a professional relationship based on trust. You work with a high degree of independence, are well-organized, and possess great ability to introduce/instruct as well as a healthy dose of humor.
