Location details Yverdon establishment En Chamard 55A 1442 Montagny-près-Yverdon Lausanne establishment Avenue des Mousquines 4 1005 Lausanne MedicalCareer level 3-10 years of experienceType of employment Part-time 80%-100% Role description As part of the development of our activities, we are currently recruiting two social assistants: one position in Lausanne and one position in Yverdon. • Conduct social situation assessments, monitoring, and social support for our center's patients, taking into account the psychiatric dimension • Work within a multidisciplinary team consisting of psychiatrists, psychologists, nurses, neuropsychologists, whose mission is to ensure integrated psychiatric and psychotherapeutic follow-up • Coordinate the patient's social support in collaboration with the center's various stakeholders and the network (CMS, CSR, SCTP, DGEJ, etc.) • Conduct educational and support interventions in concrete areas (economic, administrative, professional, relational, legal, housing search assistance, search for a place in an institution) • Assume the role of administrative referent, pending the establishment of adult protection measures by the Justice of the Peace, and collaborate with the patient to avoid loss of social benefits Your profile • Bachelor HES in Social Work, social service orientation or equivalent title • You have professional experience that allows you to work in complete autonomy.
