Pflegefachfrau/-mann mit Fallführung 50-100% # Your tasks ## • With our customers, you take over assignments in basic and treatment care • You play a decisive role in ensuring that our customers enjoy first-class and individual care at home • You advise and accompany our customers and their relatives, based on their individual needs • You carry out needs assessments with RAI-HC at the customer's location • You participate in preventive, diagnostic and therapeutic measures • Promotion of independence and quality of life of the customers in the domestic environment • You carry out comprehensive nursing anamneses and assessments • You instruct the subordinate employees with regard to nursing measures • You take over administrative tasks within the framework of case management What you bring with you ## • Training as a Dipl.
