Socio-educational Assistant or Assistant in Community Nursing and Care (F/M/D) In 6-month CDM between 60% and 80% - EPSM Le Pré-Carré La Fondation Saphir, a multi-mission and multi-site foundation (psychogeriatrics, geriatrics, adult psychiatry), is active in the medico-social field in the canton of Vaud and in the reception and support of persons whose physical and mental integrity is fragile during their life process. In order to complete our support team, we are looking for the following profile for our establishment in Corcelles-sur-Chavornay, which welcomes 18 Persons and which specializes in the field of adult psychiatry: Socio-educational Assistant or Assistant in Community Nursing and Care (F/M/D) In 6-month CDM between 60% and 80% - EPSM Le Pré-Carré Regardless of their function, the professionals of Fondation Saphir have a common will, that of supporting Persons on a daily basis to allow them to concretely experience the institutional strategy of services "like at home". That is to say, to maintain their habits as much as possible, offer an adapted response to their wishes and, through real interprofessional collaboration, allow the well-being and happiness of the Persons. Within our establishment, you support adult Persons presenting psychic disorders, taking into account their needs, their resources and their support project. You contribute to creating a securing, stimulating and benevolent environment, promoting their autonomy, their participation and their quality of life. As part of individualized support, this position is dedicated to the follow-up of a Person presenting an autism spectrum disorder (ASD) as well as a mild mental retardation. You play a central role in their reception, their integration within the establishment and their daily support. You ensure the establishment of a structuring and reassuring environment, promoting their autonomy, their inclusion and their well-being, in close collaboration with the interprofessional team. As a Socio-educational Assistant or Assistant in Community Nursing and Care, you implement the interventions defined in the individualized support project. You observe the evolution of the Person, transmit useful information to the team and adapt your support according to their needs and the set objectives. YOUR BACKGROUND - YOUR QUALITIES AND SKILLS • You hold a CFC as a socio-educational assistant (ASE) or as an Assistant in Community Nursing and Care (ASSC). • You possess experience in supporting Persons presenting an autism spectrum disorder (ASD). • You manifest a marked interest in supporting Persons living with psychic disorders in adult psychiatry. • You demonstrate empathy, patience and an attitude centered on the needs and resources of the Person. • You have a sense of organization, responsibility and know how to integrate quickly within an interprofessional team. • You appreciate interprofessional teamwork and know how to communicate in an adapted manner. • You demonstrate a sense of organization, flexibility and autonomy in your practice. • You master French (oral and written) as well as usual IT tools. • You hold a valid driver's license.
