Mental health nurse in permanent employment at 80%-100% OSAD La Fondation Saphir, a multi-mission foundation (psychogeriatrics, geriatrics, adult psychiatry), is active in the medico-social field in the canton of Vaud and in the reception and support of people whose physical and mental integrity is fragile during their life process. In this context and in order to complete our team, we are looking for our OSAD / Organisation de soins à domicile in Yverdon-les-Bains, which has 177 beneficiaries and is aimed at adults suffering from mental and/or psychological disorders whose life journey requires home support: Mental health nurse in permanent employment at 80%-100% OSAD Regardless of their function, the professionals of Fondation Saphir have a common will: to accompany People daily in order to allow them to concretely experience the institutional strategy of services "like at home". This means promoting the maintenance of their habits, offering a response adapted to their wishes and, through real interprofessional collaboration, allowing the well-being and happiness of the People. As a mental health nurse, you design, implement and evaluate preventive, therapeutic and rehabilitation nursing care services, in partnership with the People. You build a professional relationship with them inscribed in the perspective of their care project. You actively promote their global health as well as their support in the process of managing their health. Your presence in the field is essential: your interventions are part of observation, the transmission of good practices and the responsibility of your mandate. Thanks to your expertise, you are able to reflectively evaluate the physical, psychic, social and spiritual needs of the People, and to analyze situations in a systemic approach. You participate in examinations and observations related to the medical diagnosis, manage emergencies and activate emergency procedures if necessary. In coherence with your values centered on the Person, you establish a lasting relationship of trust with the People as well as with their entourage, by co-constructing a safe, respectful environment adapted to their needs. YOUR PATHWAY - YOUR QUALITIES AND SKILLS You hold a Bachelor's degree in nursing or a recognized equivalence, completed by solid experience in adult psychiatry. You possess a great sense of observation, allowing you to quickly evaluate complex situations. You work on the basis of evidence-based data, with reflective practice and an ability for in-depth clinical analysis. You are familiar with the concept of recovery, which you apply daily in your support. You have proven knowledge of the social field and the Swiss healthcare system; a minimum of 5 years of experience in outpatient care is an asset. You place the well-being of the Person at the center of your concerns, by mobilizing adapted relational and therapeutic tools. Your natural leadership, your sense of responsibility, your autonomy, as well as your ability to question yourself allow you to guarantee the quality of the expected services. You are recognized for your empathy, your listening, your flexibility, your adaptability and your organization in a demanding interprofessional environment. You know that stress, effective communication and priority management are an integral part of the profession and you approach them with mastery. You have a perfect command of spoken and written French, and use computer tools fluently (notably Carefolio). A driver's license and a personal vehicle are indispensable.
