Nurse Coordinator

Nurse Coordinator (80–100%) Proxi\-Soins is a home care organization active in the cantons of Vaud and Fribourg. In order to strengthen our coordination team, we are looking for a Nurse Coordinator at 80–100% for our site in Moudon. You will hold a central role in the organization of care, the supervision of teams, and the development of service quality, in close collaboration with management. Your main missions Coordination of care Ensure the follow-up of new patients and the organization of their care. Supervise care files and guarantee their compliance. Organize and adjust the schedules of the field teams. Ensure the continuity and quality of services provided to patients. Collaborate with doctors, hospitals, EMS, and partners in the health network. Team management Supervise and support nurses, ASSC, auxiliaries, and administrative staff. Participate in the recruitment, integration, and follow-up of employees. Organize replacements, manage absences and vacations. Participate in the development of team skills through coaching and internal training. Administrative management Supervise administrative procedures related to care services. Ensure the follow-up of evaluations, OPAS, and documents necessary for care. Guarantee the quality and reliability of data entered into patient files. Quality and development Participate in the continuous improvement of services. Contribute to the updating of internal procedures and protocols. Participate in the development of the structure and its influence within the care network. On-call duties Participation in telephone on-call duties according to a defined rotation. Coordination of urgent situations and support to teams remotely. On-call duties are carried out mainly remotely, without systematic field intervention. Your profile HES nurse diploma or title recognized by the Swiss Croix\-Rouge. Professional experience in home care or a similar field. Experience in coordination, team management, or strong motivation to evolve toward this function. Sense of organization and excellent priority management skills. Ease in human relations and communication. Spirit of initiative, autonomy, and decision-making capacity. Good command of IT tools.
